Why is being visited by a physician in the Middle Ages contribute a little change in the treatment of disease and medical care?:)

Because early doctors knew very little about what actually caused disease and illness. They were usually just going off of direct observation and sometimes even thought that smells caused disease. They didn't know much about the actual causes.
